Keeping Primary Care Connected to Patients During COVID with Telehealth and Aledade App COVID Vulnerability Worklist

When COVID-19 hit Rapides Parish in central Louisiana, Brian Clinic knew it would have to get creative about maintaining its strong relationships with its more than 1,400 Medicare patients, many of whom were at high risk of having a bad outcome if they were to contract the virus.

Thanks to the COVID-19 Vulnerability Worklist from Aledade, Brian Clinic quickly deployed a targeted, prioritized outreach program for higher-risk individuals. Their team also implemented telehealth into their established care coordination and transitional care management programs, allowing them to connect with more than 90% of its Medicare patients within weeks and helping vulnerable members of the community ‘Stay Well At Home’.
