Aledade ACO to Provide Quality, Coordinated Care to 6,000 State and Public School Employees

Bethesda, MD—Today, Aledade announced an agreement to provide coordinated care to West Virginia Public Employees Insurance Agency (PEIA) plan members. Under the three-year contract, 6,000 West Virginia state and public school employees will join Aledade’s West Virginia ACO network, keep their primary care physician, and continue to receive high-quality, personalized care.

The Aledade West Virginia ACO will take on accountability for the total health care experience of West Virginia PEIA plan members cared for by Aledade ACO practices. In this shared savings arrangement, Aledade’s local independent primary care doctors will put a greater focus on care coordination, population health management, and preventive care. Through this partnership, Aledade and West Virginia PEIA can create value in health care through high-quality care, a healthier patient population, and lower costs.

The West Virginia PEIA agreement is the first of its kind for Aledade, and the first time West Virginia has partnered with independent, primary care physicians.

New statistics show that in 2015, Aledade ACOs had a 251% average practice increase in preventive care visits and a 175% average practice increase in transitional care services, resulting in a decrease in hospital readmission rates. Aledade ACOs had a $51,000 average increase in practice revenue in 2015.
