Aledade to Expand Primary Care Physician-Led ACO Model to Nevada and Utah

Doctors in Nevada and Utah will soon have a new local partner to help keep their patients healthy. Aledade, Inc. has announced that it will form a new primary care physician-led Accountable Care Organization (ACO) in the two states. In addition, the ACO will be supported by a local partner HealthInsight, a regional health improvement collaborative. The combination of Aledade’s unique regulatory knowledge and cutting-edge technology with boots on the ground practice transformation support from HealthInsight will help doctors deliver high quality care.

The Aledade ACO has already recruited three leading family practices to join its network: St.Mark’s Family Medicine in Salt Lake City, UT; Wade Family Medicine, in Bountiful, UT; and Westside Medical Family Practice in Clinton, UT. Aledade’s local team will continue to identify and recruit family practices in Utah and Nevada throughout the spring.Aledade helps providers navigate the shifting health care landscape by providing the resources needed to provide better patient care in the new value-based health care economy. Aledade partners with independent physicians to create and manage ACOs, networks of physicians who
band together to deliver coordinated care to patients. They operate under a payment structure that rewards positive health outcomes for patients, instead of the volume of health care services delivered.

Aledade will work with physicians across Nevada and Utah to build a Medicare Shared Savings Program ACO to facilitate coordination and cooperation among providers to improve the quality of care for Medicare Fee-For-Service beneficiaries and reduce unnecessary costs. Aledade will provide its proprietary population health management and patient outreach tools to empower physicians and their staff to deliver personalized, coordinated care to their patients. “We’re excited to begin offering physicians the tools they need to deliver the highest quality personalized care to patients. HealthInsight is already part of established, local practice transformation efforts in Nevada and Utah. Aledade ACOs focus on delivering the highest-quality care to patients, with an emphasis on preventative care, high-value referral networks, customized technology, and unmatched analysis of patient claims and electronic health record (EHR) data to identify patients who need help before serious medical problems arise. Over the past year, Aledade ACO practices have increased preventative care visits by 400 percent and vaccination rates by 250 percent; as well as helped practices increase revenue for performing important preventive services.
