Local doctors in Arkansas have a new partner to help keep their patients healthy. Aledade has signed an agreement with Arkansas Office of Health Information Technology which administers the State Health Alliance for Records Exchange (SHARE), the state’s Health Information Exchange (HIE), to receive real time information, such as Admit, Discharge, and Transfer (ADT) data, on their patients.
By partnering with SHARE, providers in Aledade’s Primary Care ACO will electronically receive notifications when their patients are hospitalized. Knowing when a patient is admitted or discharged from the hospital enables providers to connect with patients during care transitions, resulting in higher rates of medication adherence and lower rates of hospital readmission. Studies show that if a patient has been contacted by their doctor within 48 hours of discharge, the patient is more likely to follow instructions and less likely to be readmitted to the hospital.
Health information exchanges like SHARE play an important role in supporting ACOs and value based care. By building just one data connection to the HIE, ACO providers can exchange information with the 26 hospitals and 317 practices on the network, rather than buying and building individual interfaces to each. This saves providers time and money, and allows them to provide more personalized, prompt care.
