We celebrate all women at both Aledade and Aledade Care Solutions every day of the year. And this International Women’s Day, we’re taking some time this week to spotlight three trailblazing Aledaders who are truly making “herstory” in health care. Get to know our first amazing woman below.
Jo Green
Meet Jo Green, Patient Outreach Specialist for Aledade Care Solutions. As her title suggests, she supports practices in the Aledade ACO network by reaching out directly to their patients – particularly those on her Wellness Worklist – to follow up with them after emergency department visits, ensure they see their physician for any and all preventative screenings and schedule them for their next annual wellness visit.
“This work is important in multiple ways,” says Jo. “Many patients haven’t seen their provider in over a year. Some patients have gone a day, a week or even a month without speaking to anyone. Often, I’ll start the call focused on helping them with their care, and the conversation will blossom into something else. They’re thankful I called – and so am I. That one phone call can make a world of a difference.”
She continues: “One time, I had a patient in Louisiana – he consistently appeared on my ED Worklist. I had spoken with him several times; he and I got to know each other fairly well. I asked him if there were any life-altering changes that I should be aware of – anything that could affect his well-being. He told me that he had recently lost his job and didn’t have a car – so he was unable to get to the practice. I told him that we were ready and able to help. The practice had a transportation service he could take advantage of, and we could talk to a nurse to get his prescriptions placed on mail order. He accepted my offers, we got him scheduled to see his primary care physician and he was on the road to improving his health – all due to the fact that we showed him empathy and compassion on the call and provided him with the specific support he needed. I followed up with him a week after his visit, and he informed me that he was feeling much better. He also told me to let him know if I was ever in the area – there’s an open invitation for a crawfish boil.”
In her role and day-to-day life, Jo goes above and beyond for both patients and her family – and today, she says she is celebrating all women for doing the same.
“We are all remarkable, but I can’t forget the women who look[ed] like me – the names and stories purposely left out of history books…names and faces we may never know. My late grandmother Ollie Mae Duncan Green lived through times when she was denied opportunities because she was Black, but also because she was a woman. There has been progress in many areas, but I am very aware that we have a lot more work to do. We have to continue to fight for the inclusion and rights of ALL women. As the great Maya Angelou once said, ‘Each time a woman stands up for herself without knowing it possibly, without claiming it, she stands up for all women.’”
