This toolkit complements the treating clinician’s judgment and decision-making with the goal of proactively addressing uncontrolled blood pressure and reducing health disparity in patients. A recent survey with practices and community health centers using our 2021 Blood Pressure Toolkits showed an improvement in the accuracy of blood pressure readings, enhanced identification of patients at hypertension risk and an increased 30-day follow-up visit rate.
