Aledade, Primaris to Create New Primary Care Physician-Led ACO

Bethesda, MD and Columbia, MO – Doctors in Missouri will soon have a new local partner to help keep their patients healthy. Aledade, Inc. and Primaris, a Missouri-based leader in healthcare quality business solutions, have announced a partnership to form a new primary care physician-led Accountable Care Organization (ACO). Partnering in the Missouri ACO will help doctors deliver high quality care by combining Primaris’ local health care expertise with Aledade’s unique regulatory knowledge and cutting-edge technology.

Aledade helps providers navigate the shifting health care landscape by providing the resources needed to provide better patient care in the new value-based health care economy. Aledade partners with independent doctors to create and manage ACOs, networks of physicians who band together to deliver coordinated care to patients. They operate under a payment structure that rewards positive health outcomes for patients, instead of the volume of health care services delivered.

Aledade and Primaris will work with physicians across Missouri to build a Medicare Shared Savings Program ACO. Aledade will provide its proprietary population health management and patient outreach tools that empower physicians and their staff to deliver personalized, coordinated care to their patients.

Primaris will provide in-person support for physicians throughout the entire setup and operation of the new organization. Primaris will help practices implement and enhance wellness services and care coordination, including after-hours services, specialists, and long-term care coordination.
