Aledade Partners with Gateway Medical Associates to form Physician-Led ACO

Bethesda, MD – Today, Aledade announced a partnership with Gateway Medical Associates to form a physician-led Accountable Care Organization (ACO) serving the communities of south-east Pennsylvania. In the Aledade ACO, Gateway Medical Associates, a physician-owned practice, will deliver high-quality, coordinated care to 6,200 Medicare beneficiaries from its eight locations across Chester and Delaware County.

Aledade offers a new model of primary care by partnering with physicians to build and lead Accountable Care Organizations (ACOs) – networks of physicians who band together to deliver coordinated care to patients. Aledade ACOs allow doctors to stay independent, practice medicine like they’ve always wanted to, and thrive by keeping people healthy. The Aledade model operates under a value-based payment structure that rewards positive health outcomes for patients, instead of the volume of health care services delivered.

Gateway implemented an electronic health record system in 2006, as well as direct interfaces with local providers like Main Line Health, Chester County Hospital, Brandywine Hospital and commercial laboratories, making all patient data immediately accessible. This enables Gateway providers to make more timely decisions regarding their patients’ care, which facilitates Gateway’s approach centering on its patients’ well-being and health. New data shows that Aledade ACOs, in 2015, had a 251% average practice increase in preventive care visits and a 175% average practice increase in transitional care services, resulting in a decrease in hospital readmission rates. Aledade ACOs had a $51,000 average increase in practice revenue in 2015.
