Aledade Expands into New Patient Populations and New Markets

Bethesda, MD— On the second anniversary of its founding, Aledade announced several key partnerships that highlight how the company is growing and expanding beyond Medicare into new patient populations, including private insurance and Medicaid.

Aledade is partnering with two new entities: Blue Cross Blue Shield of Louisiana to provide care to their members and 10 Federal Qualified Health Centers in West Virginia to provide care to low-income patients.

In addition, Aledade is expanding its geographic coverage and partnering with primary care physicians to form ACOs in Nevada/Utah, Michigan, Missouri, and Pennsylvania.

In Louisiana, Aledade has signed its first commercial contract with Blue Cross Blue Shield of Louisiana. Aledade’s physician-led ACO will join Blue Cross Blue Shield’s Quality Blue Value Partnership (QBVP) program. The Aledade ACO and its member physicians will take on accountability for the total health care experience of Blue Cross Blue Shield of Louisiana patients cared for by the ACO’s practices.

Aledade’s Louisiana ACO is made up of six leading primary care practices in Lafayette, New Iberia, Abbeville and Opelousas, providing care to over 6,000 Medicare beneficiaries and is the only physician-led ACO in Louisiana to start in 2016 through the Medicare Shared Savings Program (MSSP). Aledade’s partnership with Blue Cross Blue Shield of Louisiana will double the number of patients benefiting from the coordinated, high-quality care delivered by Aledade’s Louisiana ACO.

In West Virginia, Aledade is partnering with the West Virginia Primary Care Association and forming a new Federally Qualified Health Center (FQHC) led ACO. The Aledade West Virginia Health Center ACO brings together 10 leading health centers from across West Virginia to deliver high-quality, coordinated care to medically underserved communities in the state. It is the only health center led ACO in the state.

The Aledade West Virginia Health Center ACO will focus on care coordination, population health management, and preventive care for 13,000 patients, which will nearly triple Aledade’s patient population in the state. Aledade currently has an existing ACO covering nearly 8,000 patients.

In addition, Aledade is establishing ACOs in new markets in Pennsylvania, Michigan, Missouri, Utah and Nevada.

In Pennsylvania, Aledade is setting up two physician-led ACOs. The first ACO is in partnership with Gateway Medical Associates and will deliver high-quality, coordinated care to 6,200 Medicare beneficiaries across Chester and Delaware County in south-east Pennsylvania. The second ACO includes three local practices in central Pennsylvania: Graham Medical in Newville; Southeast Lancaster Health in Lancaster; and Waynesboro Family Medical Associates in Waynesboro, and continues to identify and recruit high-quality practices.

In Michigan, Aledade is partnering with Great Lakes Organized Systems of Care (OSC) to establish the Great Lakes Physicians ACO in mid-Michigan. The ACO is currently made up of 15 practices and will also continue to identify additional high-quality practices. The physician-led ACO will deliver high-quality care through a focus on care coordination, population health management, and preventive care.

In Utah and Nevada, Aledade has established a physician led ACO that covers both states. The ACO has expanded to nine new practices in Utah and continues to identify and recruit high-quality practices in both Utah and Nevada.

New data shows that Aledade ACOs, in 2015, had a 251% average practice increase in preventive care visits and a 175% average practice increase in transitional care services, resulting in a decrease in hospital readmission rates. Aledade ACOs have a $51,000 average increase in practice revenue in 2015.

All Aledade practices offer same or next day appointments for patients with urgent needs, and 95 percent of Aledade physician partners provide 24 hour-a-day, seven-day-a-week patient access to an on-call doctor.
