Aledade Announces New Federally Qualified Health Center ACO in West Virginia

Bethesda, MD—Today, Aledade announced it has formed a new Federally Qualified Health Center (FQHC) led ACO in West Virginia. Aledade West Virginia Health Center ACO is the only health center led ACO in the state and brings together 10 leading health centers from across West Virginia to deliver high-quality, coordinated care to medically underserved communities in the state.

The ACO expands Aledade’s footprint in the state, where Aledade has an existing ACO covering nearly 8,000 patients. The new ACO will focus on care coordination, population health management, and preventive care for 13,000 patients.

The initial health center members of the ACO include: FamilyCare Health Center; Change, Inc.; Rainelle Medical Center; Valley Health Systems; Cabin Creek Health Systems; New River Health Association; Monongahela Valley Association of Health Centers; St. George Medical Clinic; Belington Community Medical Services Association; and Clay-Battelle Community Health Services Association. Aledade will continue to identify and recruit leading health centers to expand the ACO and provide quality care to more patients.

Aledade has partnered with the West Virginia Primary Care Association in forming the Aledade West Virginia Health Center ACO.
