At Aledade, we take very seriously our mission to support and empower primary care practices. As such, the Aledade App now displays a new COVID-19 vulnerability risk score to help its partner practices keep appropriate patients healthy and home.

The App assigns a COVID-19 vulnerability category based on patient characteristics that correlate with higher morbidity and mortality from COVID-19 infection. Age and chronic conditions such as CVD, COPD, HTN, DM, and cancer are associated with higher vulnerability.
