In first two years, practices improved care for over 41,000 patients, resulting in nearly $14 million in shared savings.
JACKSONVILLE, Fla., and BETHESDA, Md. — September 7, 2023 — Florida Blue, the state’s Blue Cross Blue Shield plan, and Aledade, the largest network of independent primary care providers in the country, today announced that their collaboration on a value-based care network in Florida helped improve the quality of care and patient outcomes for tens of thousands of commercially-insured members over two years of partnership. Led by Florida physicians, the collaboration resulted in nearly $14 million in shared savings earned by this network of nearly 60 primary care practices, federally-qualified health centers, rural health clinics and multi-specialty clinics.
Value-based care networks, or Accountable Care Organizations (ACOs), are groups of providers that share financial responsibility for delivering high-quality, better-coordinated patient care. Results from the Aledade-Florida Blue ACO show the 59 primary care practices that participated in 2022 outperformed the market in rates of control of diabetes, as well as screening rates for breast cancer, colorectal cancer and cervical cancer. The practices also increased their rates of patient outreach through workflow management powered by the Aledade App, connecting with 3 out of every 4 patients following an inpatient visit. This follow-up is a critical way to enhance the relationship between these practices and the more than 41,000 patients served under this collaboration.
Additionally, between year one and year two, these practices reduced hospitalizations by twice as much as other practices in their region (a 15 percent reduction compared to 7 percent) and visits to the Emergency Department (a 1.8 percent reduction compared to 0.8 percent).
“Florida Blue values partners like Aledade that share our commitment to addressing health care access, affordability, and quality,” said Phil Lee, Florida Blue’s Vice President, Strategic Provider Partnerships. “This collaboration demonstrates the importance of partnering to address overall health and well-being while improving the experience for providers and patients, driving better outcomes, and lowering costs.”
Aledade’s work with Florida Blue enables clinicians, practices and Florida Blue to work as a team to improve the quality of care for patients, better manage chronic diseases, and ultimately improve health care affordability. As an innovative industry leader enabling healthy communities in the state of Florida, Florida Blue continues to broaden the traditional scope of care, shaping the evolution of value-based care to achieve better health for its members. This includes providing necessary claims data and support to Aledade, who then provides participating physicians with a full range of capabilities that includes cutting-edge data analytics, user-friendly guided workflows, health care policy expertise, and hands-on support from local experts in practice transformation.
“We are deeply grateful to Florida Blue for working with us to empower primary care practices in Florida and bring better primary care to tens of thousands of Florida patients,” said Ananya Banerjee, Chief Commercial Officer at Aledade. “Every patient deserves the proven benefits of physician-led value-based care, whether they get their health coverage through traditional Medicare, Medicare Advantage, commercial health plans or Medicaid. Florida Blue’s partnership and the structure of their value-based care program, including sustainable benchmarking methodology and regional benchmarking, ensure the true alignment essential to value-based care.”
The joint effort demonstrates both organizations’ commitment to health care quality, affordability and access, and to creating opportunities for primary care practices to remain financially independent.
