Aledade to Form Primary Care Physician-Led ACO in New Jersey

Bethesda, Md. [August 15, 2017] – Today, Aledade, Inc. announced it is forming a primary care physician-led Accountable Care Organization (ACO) in New Jersey. As a national leader in value-based care, Aledade will bring its innovative new model of primary care to New Jersey physicians to help them deliver high-quality care and better outcomes for the state’s patients. Aledade’s ACO in New Jersey will benefit from the support of a local health care leader, the New Jersey Academy of Family Physicians (NJAFP).

In their first year as ACOs, Aledade practices have increased pneumonia vaccinations by nearly five times, more than tripled preventive care visits, and more than doubled transitional care services for patients leaving a care facility. Readmissions and unnecessary emergency department visits fell in both of Aledade’s inaugural ACOs, and practice revenue grew by an average of 66 percent. Today, all of Aledade’s partner practices offer same or next day appointments, and 95 percent provide access to an on-call physician 24 hours a day, 7 days a week.
