Aledade Announces Growing Partnerships with Independent Physicians on Fourth Anniversary of Founding

Bethesda, Md. – Aledade, Inc., celebrated its fourth anniversary of helping independent physicians move to value-based care by announcing today that its national network has grown to include nearly 300 independent physician practices across 20 Accountable Care Organizations (ACOs) that together are accountable for more than 300,000 lives and $2.5 billion in total health care spending in 30 value-based contracts. Aledade also announced it is forming new ACOs in North Carolina, Georgia, and the greater Philadelphia area — while continuing to expand its presence in existing markets. By expanding its contracting in the Medicare, Medicare Advantage, Medicaid, and commercial insurance markets, Aledade is ensuring that practices can implement improvements across their entire patient panel.

Aledade ACOs provide patients greater access to more informed and more engaged primary care. Over each of the last three ACO program years, practices that joined an Aledade ACO have more than doubled preventive care visits and transitional care services. This has led to a continual decline in unnecessary emergency department visits and readmissions across Aledade ACOs, according to Medicare data. Additionally, patients in Aledade ACOs have seen a 7 percent reduction in total hospitalizations and a 20 percent reduction in skilled nursing facility utilization. For practices, this focus on quality improvement and preventive care has proven rewarding as every practice in all of Aledade’s ACOs earned an exceptional quality bonus payment in 2019 for their performance in the 2017 MACRA MIPS program.

To further this alignment, the company is taking on two-sided risk (MSSP Track 3) in its ACOs’ second contract periods.
